Unfortunately, we are currently at capacity for Child and Adolescent Psychiatry services and are unable to accept new referrals at this time. We recommend checking back periodically, as availability may change. In the meantime, we suggest seeking alternative services to ensure timely care and support for your child. Adult Patient intake Form (16 years of age and above)

Please complete the intake Form below and upload your GP referral along with any other relevant documents as soon as possible.

At present, all our Adult Psychiatrists are offering appointments via Zoom only. Once submitted, your intake form will be reviewed by our Practice Manager, who will assess your information and allocate the most suitable psychiatrist for your needs. Please note that completing this form does not guarantee an appointment, as there may be times when we are at full capacity or recommend alternative services.
